A case of primary biliary corrhosis and adenomatous goiter in a patient with a family history of autoimmune thyroid disease.

A 73-year-old woman with primary biliary cirrhosis (PBC) and adenomatous goiter is reported. She was hospitalized for evaluation of liver disease and diffuse goiter. Liver biopsy specimens showed chronic non-suppurative destructive cholangitis, and revealed PBC, stage II (Scheuer). Immunohistological study of liver tissue demonstrated that the majority of cells in the lymphoid follicle-like structures were helper/inducer T-cells. Histology of the thyroid after thyroidectomy revealed adenomatous goiter with only focal lymphocytic infiltration. Her sister and daughter also has a goiter. Autoantibodies were found in her family; antithyroidal antibodies in her sister, brother and daughter and anti-smooth muscle antibodies in her brother and daughter. These findings suggested the possibility that an autoimmune mechanism was associated with the pathogenesis of her adenomatous goiter, and that a common immunogenetic factor may have existed in the development of both PBC and adenomatous goiter.
