Orbital apex syndrome has commonly been considered to have a poor prognosis. However, the first choice therapy for orbital apex syndrome is well known to be a radical operation including ophthalmectomy. Recently, some hospitals have selected conservative therapy. In our hospital, a case of orbital apex syndrome was controlled by conservative therapy that included endonasal operation (surgical drainage). This case has been followed up for 6 years. If an orbit is not severely necrotic and does not have any possibility of causing sepsis, we suggest that conservative therapy including endonasal operation is preferable over an aggressive surgical operation.
