Spontaneous Intracranial Hypotension with Bilateral Subdural Hematomas in a Young Man

A 26-year-old man presented with bilateral subdural hematomas due to spontaneous intracranial hypotension (SIH). He had complained of headache during working and magnetic resonance imaging (MRI) with gadolinium showed diffuse pachymeningeal enhancement with bilateral subdural hematomas. The bilateral subdural hematomas were drained, but 18 days thereafter the left subdural hematoma reappeared. So that further surgery was necessary. Three months after the first operation, MRI demonstrated of diffuse pachymeningeal enhancement to no longer be present. In SIH with bilateral subdural hematoma patients, contrast-enhanced MRI has proved to be both safe and effective, avoiding the risk of hematoma expansion associated with examination of cerebrospinal fluid. Subdural hematomas are often bilateral in SIH with a high likelihood of recurrence, requiring careful follow-up.
