Anatomical Study of the Endoscopic Transnasal Transsphenoidal Approach for Pituitary Surgery

The anatomy of the nasal cavity, sphenoid sinus and sella turcica, as it relates to endoscopic pituitary surgery, was studied in 36 cadaver heads. Sagittal sections of 32 cadaveric heads were analyzed with regard to limitations on nasal speculum opening, the distance between the anterior nasal spine and the anterior wall of the sella, and the relationship between the wall of the sphenoid sinus and neurovascular structures. Four cadavers were directly observed the anatomy by the endoscopic transnasal transsphenoidal approach. The results of this study demonstrate the importance of the distance between the anterior nasal spine and the ostium of the sphenoid sinus. In this procedure, since the working space using a nasal speculum is narrow (1.2cm in diameter) and deep for an endoscopic transnasal transsphenoidal approach, surgeons should be very well-trained.
