Thrombolytic therapy combined with embolectomy as an ideal treatment for acute occlusion of major cerebral vessels : an experimental study

Objective : The purpose of this study is to clarify the effective treatment for main-trunk, thrombo/thromboembolic occlusion. Materials and Methods : Complete occlusion could be induced in 14 adult mongrel dogs, which were divided into 4 groups for control (n=4), intravenous (IV) thrombolytic therapy (n=3), intra-arterial (IA) thrombolytic therapy (n=3), and intra-arterial thrombolytic therapy with thrombectomy by OASIS (n=4). Urokinase was used as the thrombolytic agent with the same total dose of 120, 000U in all groups except the controls. Results : The results showed excellent recanalization to be established in the OASIS group resulting in smaller infarcts than in the IV or IA groups. No hemorrhagic complications were encountered and there were no significant differences between the IV and IA groups, although the IA group demonstated a tendency for a smaller area of infarction with better recanalization (93.5mm^2) than the IV group (112.8mm^2). Conclusions : Clinically, thrombectomy may become a feasible treatment for patients with main-trunk occlusion of cerebral vessels with a major potential to improve outcomes.
