Sheehan's syndrome with osmotic myelinolysis

Magnetic resonance (MR) images were obtained for a patient with hyponatremia due to Sheehan's syndrome with osmotic myelinolysis one week after she gave birth. The pons, deep white matter, and corpus callosum were hyperintense on T2-weighted images, fluid-attenuated inversion recovery images, and diffusion-weighted images. Areas that were abnormal in T2-weighted images showed lower apparent diffusion coefficient values than other normal areas. These findings were consistent with restricted water diffusion and increased intracellular water, without significant neuronal damage. The patient recovered promptly after a normal serum sodium level was achieved, and the MR aberrations disappeared. This is an informative case of Sheehan's syndrome in which we obtained MR images in the acute stage and for which we discuss possible pathophysiological mechanisms.
