Experimental photodynamic therapy with 5-aminolevulinic acid-induced protoporphyrin IX for malignant glioma

Objective : The potent photosensitizer protoporphyrin IX (PPIX) accumulates specifically in tumor cells when 5-aminolevulinic acid (5-ALA) is administered. The effect of 5-ALA-based photodynamic therapy (PDT) for brain tumors was investigated using a rat model. Materials and Methods : Twenty-seven rats underwent craniotomy and transplantation of 6C-glioma cells. The rats were divided randomly into the following groups : a non-PDT control group (N = 9), 5-ALA-excluded 640 nm, 670 nm, and combined 640 nm and 670 nm PDT groups (N = 9), and 5-ALA-based 640 nm, 670 nm, and combined 640 nm and 670 nm PDT groups (N = 9). In each 5-ALA-based PDT group, 30 mg of 5-ALA was administered intraperitoneally for 3 hours prior to irradiation. In all PDT groups, a total laser dose of 100 J/cm^2 was irradiated at a dose-rate of 100 mW/cm^2 7 days after tumor transplantation. The dose-rate of the two components of the combined 640 nm and 670 nm irradiation was 50 mW/cm^2 respectively. Seven days after irradiation, the brains were removed, and histological tests were performed. Results : The range of necrosis in the 5-ALA treatment group (N = 9) and the non-5-ALA treatment group (N = 9) was 1.0-2.6 mm (mean 1.9 mm), and 0.4-0.6 mm (mean 0.5 mm), respectfully, showing extensive necrosis in the deeper brain areas of the 5-ALA treated group. A significant difference due to type of laser wavelength could not be seen. However, with the 640 nm laser irradiation alone, necrosis tended to be more widespread in the deeper areas compared to irradiation of the 670 nm laser alone or the combined 640 nm and 670 nm lasers. Conclusion : 5-ALA-based photodynamic therapy may be an effective therapeutic method for brain tumors, however further investigation is warranted.
