Lipoprotein(a) Levels in Coronary Artery Disease Were Elevated but not Associated with Diabetes Mellitus

We studied the influence of diabetes mellitus (DM) on lipoprotein (a) [Lp(a)] levels in patients with coronary artery disease (CAD). We divided 132 patients aged 41 to 87 years into 4 groups as follows; N-group: DM (-) CAD (-), C-group: DM (-) CAD (+), D-group: DM (+) CAD (-), DC-group: DM (+) CAD (+). On coronary angiography, patients with CAD were defined as those havin≧75% fixed stenosis in major coronary arteries. DM was defined a≧140 mg/dl of fasting blood glucose or≧200 mg/dl of spontaneous blood glucose. Lp(a) in the D-group was not higher than in the N-group (15.5±2.2 mg/dl versus 12.1±1.7 mg/dl, ns). Lp(a) in patients with CAD (C-group; 25.6±4.4 mg/dl, and DC-group; 27.8±3.8 mg/dl) was higher than in those without CAD (N-and D-group) (N-group vs. C-group; p<0.01, D-group vs. DC-group; p<0.01). Furthermore, we performed a multivariate analysis to know what factors affect the presence of CAD, using the stepwise forward selection method. Identified variables were HbA_1C, Lp(a), fasting blood glucose, apo AI, total cholesterol and apo E as significant (F>2.0) risk factors for CAD. Especially Lp(a) was most valuable both in diabetic and non-diabetic patients. In conclusion, our data suggested that plasma Lp(a) was elevated in patients with CAD but not affected by DM, and multivariate analysis disclosed that Lp(a) level was most valuable in the diagnosis of CAD.
