Spontaneous Cerebrospinal Fluid Leakage Detected by Magnetic Resonance Cisternography : Case Report

症例は49歳の男性で特に誘因なく鼻漏が出現し、グルコース陽性で髄液鼻漏と診断された。CT脳槽造影で漏出部の同定は不可能であったが、3次元CTで左中頭蓋底に2個の小孔を認めた。冠状断bone window CTで蝶形骨洞は異常に外側に進展し、MR脳槽造影で小孔から蝶形骨洞への髄欣の漏出が確認できた。術中同部位で硬膜は網目状に罪薄化し骨順に陥入しており、筋膜で修復した。術後鼻漏は消失した。特発性髄液漏の手術は漏出部をいかに正確に同定するかが重要で、これには3次元CTによる頭蓋底の検索とMR脳槽造影での頭蓋内外の連続性の確認が必須である。また本例の如く蝶形骨洞の外側進展が重要な所見である事を強調した。
