Ten patients were preoperatively intrathecal administered 100μg morphine for postoperative analgesia after laparoscopic cholecystectomy in comparison with the same number of patients postoperatively administered 15mg pentazocine intramuscularly. Pain intensity, assesed using the visual analogue scale (VAS), in the intrathecal morphine group was 7 (0-20) mm immediately and 8 (0-55) mm at one day postoperatively, whereas that in the intramuscular pentazocine group was 73 (22-91) mm and 44 (25-58) mm, respectively (median (range)). There were significant differences between the intrathecal morphine group and intramuscular pentazocine group both immediately and one day postoperatively. Blood pressure, pulse rate and respiratory rate did not change during this study, either in the intrathecal morphine or intramuscular pentazocine group. There were no remarkable complications due to intrathecal morphine administration even after surgical laparoscopy with CO_2 insufflation. In this study, extremely low-dose intrathecal morphine administration was found to be a useful and safe method of postoperative analgesia for laparoscopic cholecystectomy.
