The antinociceptive effects of intrathecal coadministrations of midazolam (benzodiazepines) and diltiazem (calcium channel blockers) were investigated in rats. Antinociceptive effects were assessed with the tail-flick test. Although higher intrathecal doses 30μg of midazolam produced an antinociceptive effect, 10μg did not affect the noxious response. When similarly administered, diltiazem alone (100 or 200μg) did not produce an antinociceptive effect. However, concomitant administrations of noneffective doses of midazolam (10μg) and diltiazem (200μg) elicited significant suppressive effects on the thermonociceptive response. Furthermore, this suppressive action by noneffective doses of midazolam and diltiazem was significantly reversed by the benzodiazepine antagonist flumazenil (20μg). It is therefore considered that diltiazem may significantly elicite the antinociceptive effect of midazolam. These findings suggest that the calcium channel plays an important role in the antinociceptive effects of midazolam.
