To clarify the mechanism of acute re-expansion pulmonary edema with respect to hydrostatic pressure changes, we measured the hemodynamic variables and lung water before and after the re-expansion of the experimental unilateral pneumothorax in dogs. Unilateral pulmonary edema was observed in four dogs out of seven dogs, whose PaO_2 was less than 50 torr ; colloid osmotic pressure decreased when the lungs were collapsed and lung re-expansion was performed with negative pressure. By applying the results to Starling's equation, it is concluded that the cause of pulmonary edema in our model is an increased pulmonary capillary pressure and decreased interstitial pressure. The decreased colloid osmotic pressure may act as the secondary factor.
