The paratracheal approach for stellate ganglion block has been recommended as the safest route because of its technical simplicity and lack of complications. We experienced a rare complication of inadvertent subarachnoid injection of local anesthetic during stellate ganglion block.A-42-year old female received stellate ganglion blocks for relief of tinnitus. Soon after the sixth stellate ganglion block with 8ml of 1% lidocaine, she complained of numbness of the arm. A few minutes later she became cyanotic, her respiration ceased, her femoral pulsation was not palpable, and there was no light reaction.All resuscitative maneuvers were initiated. Spontaneous respiration appeared 50 minutes later and conciousness was regained one hour after the onset of suscitation.Clinical findings suggested inadvertent subarachnoid injection of lidocaine. The possible cause of the spread of local anesthetic into the subarachnoid space are discussed.
