A case of syncopal attacks during glossopharyngeal neuralgia

We studied the mechanism of syncope during glossopharyngeal neuralgia. The patient was a 76-year-old man who periodically suffered from a sharp pain below his left ear when opening his mouth or swallowing for the past 8 years. He was admitted to our hospital because of syncopal attacks associated with pain. The electrocardiogram during such pain demonstrated cardiac arrest of a 5-second duration. Brain magnetic resonance imaging (MRI) showed compression of the left glossopharyngeal nerve by the left posterior inferior cerebellar artery. Bradycardia was improved after a pacemaker implantation but pain and hypotension continued. After treatment with carbamazepine both pain and syncopal attacks disappeared. Carbamazepine was effective for glossopharyngeal neuralgia due to compression by cerebral arteries. MRI is useful to evaluate glossopharyngeal neuralgia with syncopal attacks.
