A case of brainstem abscess due to Nocardia in a healthy 48-year-old man

Solitary brain stem abscess is a rare disorder and often makes differential diagnosis difficult. We experienced a solitary brainstem abscess due to Nocardia infection in a 48-year-old man who had been previously healthy. He was admitted to the hospital complaining of fever, headache, dysesthesia and weakness on left extremities which had developed over the past 10 days. An admission diffusion-weighted MR image demonstrated hyperintensity on the right dorsal pons, but enhanced T1-weighted MR image did not show ring enhancement. This case emphasized the importance of diffusion-weighted MR images for establishing diagnosis of solitary brainstem abscess. No pathogens or organisms were cultured in the specimens. However, from the microscopic exam, we discovered the pathogen was Nocardia, an attenuated pathogen that may cause brainstem abscess in patients without definable predisposing conditions.
