Factors determining self-rated health : survey on evaluation of health status

Objectives : Recently, life-style-related diseases like hypertension and diabetes account for a large proportion of diseases burden of our country, often being left untreated because they are asymptomatic and patients' compliance with treatment is low. This is because some people are unable to self-evaluate their own health correctly. Self-rated health is widely used for evaluation of the general population's heath. However, it can be affected by individual factors. This study aimed to clarify how people evaluate their health status and how individual factors affect "Self-rated health." Materials and Methods : We conducted a questionnaire survey on health for a total of 4, 240 people, all residents over 20 years old in Aoki village in Nagano Prefecture, Japan, in March 2005. For evaluation of self-rated health, we adopted the questionnaires, including eight domains of health, developed for the World Health Survey (WHS). Out of 2, 504 respondents, 556 subjects who had received the annual health checkup related to cardiovascular diseases in the previous year were employed for the analysis. Results : There were no relationships between the subjects' self-rated health and either their blood pressure levels or the comprehensive results. The results of the multiple regression analysis with the self-rated health as a dependent variable, and individual factors and the results of health checkups as independent variables, showed that "Mobility, " "Pain, " and "Affect, " out of the eight domains, and some individual factors affected the evaluation of self-rated health. On the other hand, the results of the health checkups did not correlate with self-rated health. Conclusions : The results of health checkups do not affect the self-evaluation of health. However, subjective factors such as "Mobility, " and "Pain" affected the evaluation of self-rated health, showing that people do not rely on the health checkup results alone to evaluate their health status. Thus, people without any health problems that can be recognized tend to over evaluate their health status, and tend not to go to a clinic even if they had some significant problems found in health checkup. Health education for people who over evaluate their health is necessary so that they can recognize their health status appropriately.
