Diagnostic efficacy of superparamagnetic iron oxide enhanced MR (SPIO-enhanced MR) imaging for the effectiveness of radiofrequency ablation for hepatocellular carcinoma : comparison with helical CT

Background : Accurate diagnostic assessment of therapeutic efficacy is important for hepatocellular carcinoma (HCC) after radiofrequency ablation (RFA). We assessed the diagnostic accuracy of superparamagnetic iron oxide enhanced magnetic resonance imaging (SPIO-enhanced MRI) after RFA for HCC compared with computed tomography (CT). Materials and Methods : A total of 127 patients with 150 HCC underwent CT and SPIO-enhanced MRI before and within 7 days after RFA. The outcome was assessed in 122 nodules which were followed up more than 6 months after treatment. The diagnostic accuracy of SPIO-enhanced MRI and CT was evaluated by cumulative recurrence rate. Results : Group A (diagnosed as complete ablation on both SPIO-enhanced MRI and CT) : 112 nodules; Group B (complete ablation on only SPIO-enhanced MRI) : 6 nodules; Group C (complete ablation on only CT) : 22 nodules; and Group D (incomplete ablation on both) : 10 nodules. Overall recurrence rate was 37.7%. Recurrence rates of groups A-D were 33.7%, 0%, 60.0%, 100%, respectively. Sensitivity, specificity, and accuracy of CT were 98.7%, 4.3%, 63.1%, respectively, and those of SPIO-enhanced MRI were 92.1%, 23.9%, 66.4%, respectively. Conclusion : SPIO-enhanced MRI was superior to CT in evaluating treatment outcome of HCC after RFA.
