Two separete studies on the reactivity of HLA antisera with lymphocytes from patients with gastric carcinoma are reported. In the first study, 106 HLA antisera were tested against lymphocytes from 31 to 167 patients with gastric cacer, and 34 to 283 normal healthy controls from the Kanto and Kansai districts of Japan were analysed. Significantly different (x^2 3.84) frequencies of reaction were demonstrable in 20 out of 155 comparisons made (11 showed more reactivity than patients and nine less reactivity than patients). In the second study HLA-B18 was shown to increase, with a frequency of 4.6% in a series of 87 patients, compared with 0% in the control population (x^2=11.7, p<0.0009). In addition, there was significant excess of patients with a blank allele at the A and B locus and a significantly decreased frequency of antigens in B5, BW35 and BW16. The most frequent haplotypes were A9-B5 in controls and A2-Blank in the gastric cancer patients.
