A Multi-institutional Study on Adjuvant Chemotherapy for the Prevention of Post-operative Liver Metastases in Colorectal Cancer Patients : Comparison Between Hepatic Arterial and Portal Adriamycin Plus Oral UFT Administration

In order to clarify which is more effective for the prevention of post-operative liver metastases in colorectal cancer patients, adjuvant chemotherapy consisting of hepatic arterial or intra-portal adriamycin (ADM) plus oral UFT administration was examined as an open trial of multi-institutions. Patients were given ADM 20 mg/m^2 via the hepatic artery in intra-arterial group (IA group) and the portal vein in intra-portal. Group (IP group) just after the resection of cancer lesions, respectively. Thereafter, all of the patients were orally treated with UFT 270 mg/ m^2/day for 2 years. Forty-five colorectal cancer patients with curability A and B, 12 patients in the IA group and 33 in the IP group, were enrolled in the study. No statistical difference was found in 12 clinicopathological variables between the groups. Five-year survival rate was 80.8% in the IA group and 89.9% in the IP group. Although various types of post-operative metastases were found in 25.0% in the former and 21.2% in the latter, the liver metastasis was not found in the IA group. The metastasis was found in 5 patients of the IP group. There was no statistical difference in the survival and liver metastatic curves or rates between the groups. These results suggest that the intra-arterial adjuvant chemotherapy for colorectal cancer patients is not beneficial to the prophylaxis of post-operative liver metastases, in comparison with the intra-portal chemotherapy.
