Eight cases were examined to assess the usefulness of three-dimensional (3-D) CT imaging of the main pancreatic duct following endoscopic retrograde pancreatography-helical CT (ERP-HCT), Abdominal HCT including the pancreas was taken before and after ERP. The optimal time for the highest contrast enhancement in the main pancreatic duct (MPD) and for the maximal delineation of 3-D CT in each part of the pancreas was then analyzed. The results indicated that the maximal CT value to separate the MPD from the pancreatic parenchyma was 15 min after ERP. The CT value of MPD was significantly increased by ERP from 45.38 H.U. to 424.38 H.U. Although the quality of the 3-D images following ERP-HCT was sufficient in the uncinate process, the head, the body, and the tail, the delineation of smaller branches was occasionally poor due to excretion of the contrast medium. The visualization of MPD by 3-D CT followmg ERP-HCT was satisfactorily obtained in all 8 cases. It was also shown that this modality was both qualitatively and quantitatively far better than that obtained by conventional CT in terms of the continuity and stereoscopic picture of the MPD. The method presented here can serve as a powerful tool for the diagnosis of various pancreatic disorders.
