APPLICATION OF IMMUNOCYTOCHEMISTRY TO THE CYTOLOGIC STUDY OF PERITONEAL FLUIDS IN PATIENTS WITH OVARIAN CANCER

卵巣癌に対する腹腔内細胞診は, 臨床進行期の決定や再発の診断には不可欠の手段であるが, 臨床の実際においては, しばしばその診断に迷う症例に遭遇する。これに対する新たな診断法として, 複数の抗体を用いた酵素抗体染色の応用につき検討した。使用した抗体は抗CA-125, 抗CA19-9, 抗CEA, 抗EMA抗体である。良性疾患より採取した腹水細胞診での中皮細胞の陽性率は, CA-125: 39.4%, CA19-9: 12.1%, CEA: 0%, EMA: 6.1%.卵巣癌における同一症例での組織切片と捺印細胞診とのこれら4種類の染色での染色性の一致率は91.8%, 組織切片とPap.陽性腹水細胞診との一致率は82.9%, CA-125染色を除いた3種類の染色での一致率は90.3%となった。以上の結果より, CA-125染色は悪性細胞に対する特異性が低いため臨床応用できないこと, CEA染色はその特異性が極めて高いこと, CA19-9, EMA染色は良性細胞に対する反応性はある程度みられるが, 悪性細胞に対しても高い特異性のあることが判明した。卵巣癌は組織型によりこれらの抗原の発現が大きく異なることから, CA-125を除く三つの抗体を同時に用いた腹水細胞診酵素抗体染色は, その組織切片での反応性も加味した判定を行えば, 従来の診断法に加えた新たな診断法として, 極めて有効な手段であることが判明した。
