A case of membranous nephropathy associated with renal hypouricemia

A 44 years old man was found to have a low serum urate concentration (0.7mg/dl). CuA /Ccr was markedly increased(40.4%), and was not decreased after administration of pyrazinamide, and was not increased after administration of benzbromarone. He was diagnosed as having nephrotic syndrome due to membranous nephropathy. Light microscopy revealed hyaline degeneration of the proximal renal tubular cells. No other renal tubular or metabolic abnormalities were detected. Renal hypouricemia in the present case was considered to be caused by the failure of the tubular presecretory reabsorption of urate. Proteinuria with membranous nephropathy might injure the proximal renal tubular cells, resulting in abnormal urate transport.
