Pharmacokinetics and clinical evaluation of cefoperazone (CPZ) in chronic suppurative otitis media.

1) Two grams of CPZ were administered intravenously in patients with chronic suppurative otitis media. Serum levels of CPZ 60, 90, 120 and 150min. after injection were 124.22±11.90 (n=8), 106.25 (n=1), 59.38 (n=1) and 71.88μg/ml (n=1), respectively. CPZ levels in the otorrhea fluid 60, 90, 120 and 150min. after injection were 5.93±2.24 (n=8), 18.5 (n=1), 16.41±5.47 (n=2) and 5.09μg/g (n=1), respectively.2) The clinical efficacy in 7 patients treated with 2g CPZ for 2 to 9 days was 57.1%. Mild diarrhea in one patient was the only side effect noted.3) The MIC80 for organisms frequently isolated from patients with chronic suppurative otitis media were: P. aeruginosa 12.5μg/ml, Proteus sp. (Indole positive) 6.25μg/ml, P. mirabilis 0.78μg/ml, S. aureus 25μg/ml.4) CPZ was effective in the treatment of chronic suppurative otitis media except when CPZ-resistant organisms were isolated.
