The clinical study of somatosensory evoked pontentials (SEP) caused by trigeminus nerve stimulation.

For the purpose of the clinical application of somatosensory evoked potential (SEP) as an objective examination, we studied the triggered mechanical stimulation on the trigeminus nerve region. The experiments were carried out with sixteen healthy adults (8 males and 8 females) from eighteen to twenty-four years old and seven patients who suffered from mandibular bone diseases. We applied tactile stimulation to the lower lip with a plastic bar. The trial was repeated forty times to get the summated evoked potential.<BR>In the healthy group, SEP was characterized by ten peaks. Those were named P<SUB>1</SUB> (positive peak), N<SUB>1</SUB> (negative peak), through N<SUB>5</SUB>. The latencies of those peaks were approximately 18 msec (P<SUB>1</SUB>), 56 msec (N<SUB>1</SUB>), 80 msec (P<SUB>2</SUB>), 112 msec (N<SUB>2</SUB>), 135 msec (P<SUB>3</SUB>), 177 msec (N<SUB>3</SUB>), 142 msec (P<SUB>4</SUB>), 285 msec (N<SUB>4</SUB>), 334 msec (P<SUB>5</SUB>) and 399 msec (N<SUB>5</SUB>). The latency of PI developed a tendency to be shortened by the magnitude of the stimulation. The amplitude of P<SUB>1</SUB>-N<SUB>1</SUB> was 5.7μV with 1/8g stimulation and 10.8μV with 1/2g stimulation.<BR>In the disease group, we recorded the SEP before and one month after the operation. At the preoperativ SEP, however sensory disturbances were not recognized, we noticed the abnormal findings which were an irregular wave and the delay of latency on the affected side. But there was no abnormality on the non-affected side. We acquired the results: the intraosseous lesions (ameloblastomas and mandibular cysts) that existed near the inferior alveolar nerve made the SEP's wave irregular and especially prolonged the latency of the P<SUB>1</SUB>. These findings were also recognized in the hypoesthesia cases; the inferior alveolar nerve was affected by the operation and trauma. These findings increased with the up grading of the degree of hypoesthesia. In the case of sensory paralysis, we could not record an SEP wave.<BR>Finally, we concluded that the SEP was a useful method to examine nerve disturbance in the trigeminus nerve region.
