A case of ulcerative colitis associated with barium granuloma of the rectum.

A case of ulcerative colitis complicated by barium granuloma of the rectum was reported. A 60 years old female complaining of diarrhea of three and a half years duration was referred to our department. Abdominal plain x-ray film on admission showed a radioopaque shadow (3.5×2.5cm) in the pelvic cavity. On endoscopic examination, adding to the findings of ulcerative colitis, a white-yellowish macula like lesion was seen on the left wall of the rectum 4cm above the anus. Histological findings of the biopsied specimen of the lesion showed many macrophages which phagocyted the crystal material. From the re-evaluation of barium enema examination performed previously, a diagnosis of barium granuloma of the rectum was made and it was due to the injury of the mucosa caused most likely by over-inflation of the enema tube balloon.<BR>Only eight cases of barium granuloma of the rectum have been reported to date in Japan, and there was no case associated with ulcerative colitis. In four of eight cases, surgical procedure was done since barium granuloma was diagnosed as a malignant lesion of Borrmann 2 or Borrmann 3.
