Effects of Perioperative Ulinastatin Treatments in Patients Undergoing Partial Hepatic Resection

We administered ulinastatin to 16 patients, on whom we performed partial hepatic resection, in the perioperative period to measure the levels of granulocyte elastase (PMNE), inflammatory cytokines (IL-6 and IL-8) and urinary trypsin inhibitor (UTI) for a week after operation. PMNE remained lower in the ulinastatin-treated group (Group U) than in the non-treated group (Group C) after operation; on the 3rd postoperative day, the levels were significantly lower in Group U, indicating that ulinastatin suppressed excessive activation of neutrophils. IL-6 reached its peak on the 1st postoperative day, and remained lower in Group U than in Group C. Both IL-6 and IL-8 tended to be lower after administration of ulinastatin. Administration of ulinastatin prevented increased levels as well as persistent high levels of endogenous UTI. Thus, it appeared that administration of ulinastatin in the perioperative period of hepatic resection was effective in preventing systemic inflammatory response syndrome (SIRS) or occurrence of organ impairment.
