A case of phlebosclerotic colitis : vasculitis as a possible origin

Phlebosclerosis of the mesenteric vein is a rare condition causing ischemic colitis.We report a case of a 59-year-old Japanese woman who presented with symptoms of ileus.Abdominal x-rays revealed tortuous threadlike calcification of the ascending and transverse colon.Laboratory data demonstrated no signs of any autoimmune condition or liver cirrhosis except for a mild increase of transaminase.Subtotal colectomy was performed, and the colonic mucosa was reddish, edematous with mural thickening.Histologically, dense fibrosis was observed in the lamina propria and submucosa, and the venous walls showed hyalinous thickening with occasional calcification and ossification resulting stenosis.Occasionally, inflammatory infiltration was observed in the intima.Although severely affected veins in the cecum and the ascending colon showed some degree of destruction of the elastic lamina, there was only reduction of smooth muscle in the hyalinous fibrotic venous walls in the transverse colon.Several possible causes of the disease have been proposed in the literature.However, since inflammatory infiltration is occasionally found in the intima as an early change, venous vasculitis, initially starting from intimal inflammation, may be considered as the cause leading to phlebosclerosis and eventual ischemic colitis, although the essential cause of the vasculitis remains unclear.
