A Case of Interventional Treatment of a Benign Biliary Stricture after Hepaticojejunostomy

A 73 year-old man was admitted to our hospital with abdominal pain. A 56 years of age, he was performed cholecystectomy, choledochotomy and T-tube as treatment for choledocholithiasis. Cholangitis recurred after the operation. At 71 year-old of age, the patient was underwent hepaticojejunostomy due to a postoperative benign stricture of the biliary duct. Upon admission, computed tomography (CT) demonstrated dilatation of the intrahepatic biliary tree and a biliary stone on the distal side of the anastomosis of the hepaticojejunostomy. Percutaneous transhepatic biliary drainage (PTBD) was performed. The PTBD tube was expanded to 12 Fr in diameter, and we performed percutaneous transhepatic cholangioscopy (PTCS). The calculus in the intrahepatic duct was eliminated. We dilated the anastomotic stricture using an endoscopic papillary dilatation balloon. Then, we successfully performed internal biliary drainage using a silicone tube stent, buried under the skin. The patient was discharged from the hospital in January 1998. Six months after discharge, he was well and his blood chemistry was normal. The subcutaneous drainage tube was removed in August 1998. The patient was doing well at the time of last follow up in July 2000. Therefore, a tube stent appears to be very useful for treatment of benign biliary stricture after hepaticojejunostomy.
