Pancreatic alpha fetoprotein-producing acinar cell carcinoma : An autopsy case report

Objective : An alpha fetoprotein (AFP)-producing acinar cell carcinoma (ACC) in the tail of the pancreas of a 60-year-old man is described. Material and Methods : At autopsy, the whole pancreas was replaced by the tumor, and pulmonary, hepatic, adrenal and systemic lymph node metastases were identified. Results : Histological examination revealed a markedly, solid proliferation of tumor cells with round nuclei and polyhedral, slightly acidophilic cytoplasm. No squamoid corpuscles were recognized. Immunohistochemistry showed positivity for AFP, carbohydrate antigen 19-9 (CA 19-9), trypsin and lipase for tumor celles. Tumor cells were negative for pancreatic hormones, chromogranin A, neuron specific enolase and synaptophysin. Electron microscopical examination revealed granules of variable sizes in the tumor cells, which were considered to be zymogen granules. Conclusion : For differential diagnosis of ACC including islet cell tumors and pancreatoblatomas, immunostaining and the observation of ultrastructural features were helpful.
