Orthodontic treatment of patients with unilateral cleft lip and palate using osseointegrated dental implants in conjunction with secondary bone grafting

This case report describes two unilateral cleft patients who needed maxillary lateral expansion. The bony defect in the alveolar ridge restricts orthodontic accomplishment, necessitating some kind of prosthesis for stabilizing the maxillary width and making up the discontinuity of the dental arch. A contemporary goal of cleft care is to achieve esthetic and functional improvement. After orthodontic treatment in our cases, the patients were treated by means of late secondary bone-grafting to the cleft region followed by the insertion of osseointegrated implants. Follow-up evaluation revealed good stability of the arch width and the grafted bone. The advantage of osseointegrated dental implantation in conjunction with secondary bone grafting is that it can achieve superior aesthetic appearance of the alveolar process and dental maxillary arch, while keeping the neighboring teeth intact. The results reported here suggest that the present procedure of treatment might contribute not only to esthetics but also to retention of the arch width and stability of the occlusion, in cleft patients who need maxillary lateral expansion.
