転移性胸椎腫瘍に対する total spondylectomy の経験 (Combined lateral extracavitary and posterior approaches)

We report a case of total spondylectomy by combined lateral extracavitary and posterior approaches for a metastatic thoracic spine tumor. A 49-year-old female suffered from breast cancer about two years ago. Her postoperative course was uneventful after mastectomy until last summer, when her gait gradually deteriorated and she became bedridden. Magnetic resonance (MR) imaging indicated a metastatic spine tumor which had invaded vertebral bodies, pedicles and laminae, and compressed the spinal cord circumferentially at the Th10-11 levels. Since the primary lesion was well controlled without other metastasis, total resection of the thoracic spine lesion was planned. Combined lateral extracavitary and posterior approaches were used for the total spondylectomy, since total removal of the tumor was possible under direct vision in a single stage without changing the patient's position. Vertebral reconstruction was performed using a titanium mesh cage spacer, and instrumented by vertebral screws and rods with a Texas Scottish Rite Hospital spinal fixation system (TSRH) between Th9 and Th12. Postoperatively, her neurological status improved and she became ambulant. Total spondylectomy by combined lateral extracavitary and posterior approaches is a useful method for solitary metastatic thoracic tumor when the primary lesion is well controlled.
