Rupture Mechanism of a Thrombosed Slow-growing Giant Aneurysm of the Vertebral Artery : Case Report

1剖検例を通して血栓化巨大椎骨動脈瘤の増大機序を考察した。症例は血栓化巨大椎骨動脈瘤により脳幹圧迫症状を呈する76歳の男性である。壁内出血のため徐々に増大し、最終的にクモ膜下出血のため死亡した。剖検では動脈瘤壁は形成時期の異なる層状配列した血腫で形成されており外側程新鮮であった。またdistal neck近傍の陳旧性血腫内には親動脈と連続し新鮮な血栓が充満した多数の裂け目が見られ、その裂け目は最外層の新鮮な血腫とも連続していた。しかし、血管内皮は認めなかった。以上のことから、血栓化巨大椎骨動脈瘤の増大機序の1つとしてdistal neck近傍の陳旧性血腫部から生じる動脈瘤壁解離を考えた。
