CARCINOMA IN SITU RECURRENCE RATES AFTER VARIOUS MODALITIES OF TREATMENT

• 1954年から1976年までの22年間に米国シカゴ大学病院において1,294例の子宮陸部上皮内癌症例を種々の方法で治療Lた.その後の再発率は単純子宮摘除術後症例は0.7%(455例中3例)。3例はすべて5年以上経過した後の再発で,すべて浸潤癌であった.治療の目的での円錐切除後の再発は6.4%(140例中9例)であった.しかし,円錐切除術後はじめの1年間に細胞診で十分 follow upした症例の3年以内の再発率は3.3%(127例中4例)であった.一方911例の単純子宮全摘除術を施行した症例のうち,円錐切除を先行した後子宮全摘除術を行った症例は444例あり,円錐切除から子宮全摘除までの既刊と上皮内癌の残存率には,はっきりした減少傾向はみられなかった.またこの期間に少なくとも1回細胞診を受け,上皮内癌が残存していたにもかかわらず,細胞診上,陰性としか読めなかった症例が3.5%に認められた.これらのことより,円錐切除後の再発は,殆ど,上皮内癌残存によるものであると考えられると同時に,円錐除後に,細胞診を施行すれば,残存せる上皮内癌をほとんど検出できると思われる.
