EFFECTS OF ESTRIOL VAGINAL TABLETS ON THE EPITHELIUM OF THE PORTIO VAGINALIS

The effects of locally applied estriol vaginal tablets on the epithelium of the portio vaginalis of menopausal women were studied morphologically by colpomicroscopy. Colpomicroscopy revealed that marked morphologic changes occurred in the epithelial cells of the portio vaginalis due to hypofunction of the ovaries in parallel with menopause; the cell nucleus count per unit area gradually increased with menopause as a turning point, associated with increased denseness of cell nuclei, decreased karyopyknosis, and obscure cell borders. These changes reached their acmes seven years after menopause, and remained stable thereafter. These changes were paralleled by atrophic thinning of the epithelium of the portio vaginalis and also by cytodiagnostic findings of the vaginal contents, especially by a drop in estrogen index, thus making it possible to estimate estrogenic activity of postmenopausal women by colpomicroscopic diagnosis. Colpomicroscopy disclosed that in parallel with multiplication of the epithelial layers of the portio vaginalis and hypertropy of the opithelium with daily administration of the estriol vaginal tablets, the cell nucleus count per unit area decreased, the cell nuclei were less densely packed, the degree of karyopyknosis increased, and the cell borders became clear. These changes were paralleled by cytodiagnostic findings of the vaginal contents, especially by an elevation of estrogen index. Similar changes to those presented above were observed upon administration of a long-acting estriol preparation. Since, however, the administration of the vaginal tablets was not followed by an elevation of estriol levels in the blood as observed after the administration of the long-acting preparation, it was implied that the effects of the vaginal tablets were derived from their potent local actions.
