The purpose of the present study was to clarify the role of coronary collaterals in preventing left ventricular aneurysm formation. The subjects were 18 patients with their initial episode of acute anterior myocardial infarction whose coronary arteries remained at TIMI grade 0-2 at both the acute and chronic phases. Coronary collaterals at the acute phase were unrelated to the size ratio (%) of the left ventricular aneurysm, but at the chronic phase, the size ratio of the aneurysm in patients with good collaterals was smaller than in patients with poor collaterals (5±14% vs 31±22%, p<0.05). In conclusion, the existence of sufficient coronary collaterals at the chronic phase helps to reduce left ventricular aneurysm formation.
