We are now trying to develop a more effective method of minimizing discomfort and descending blood pressure to be experienced during hemodialysis. Therefore, we have studied fluid removal method with negative and positive pressure and compared removed fluid volume, hematocrit and dialysis effect. No difference in frequency rate of discomfort and dialysis effect was noted regardless of the ratio between positive and negative pressure. No evident correlation was recognized among removed fluid volume, hematocrit and frequency of discomfort. We report the experience that a fine needle (17G) is usable for the maintenance of hemodialysis with positive pressure in ordinary cases and also experiences of the effective hemodialysis performed with low fluid removal pressure for which a high-flux membrane dialyzer was used in severe cases in which the ultrafiltration was difficult owing to the hypotension or advanced age of patient.
