Importance of Long-Term Follow-Up after Pulmonary Vein Isolation: A Study Using Event-ECG Monitors

We have been reported that event-ECG monitors are very useful for detecting the early recurrence of atrial fibrillation after pulmonary vein isolation (PVI). However, the importance of long-term follow-up after PVI using the event-ECG monitors is still equivocal. <B>Methods:</B> Consecutive 128 patients with drug-resistant paroxysmal/persistent atrial fibrillation (PAF/PersAF) who underwent PVI were enrolled; 96 PAF and 32 PersAF patients. All patients were given an event-ECG monitor before, 1-month, 6-months, and 12-months after the PVI (Control, 1M-, 6M-, and 12M-PVI, respectively). Holter-ECG monitor was also performed at 6M-PVI. <B>Results:</B> (1) Holter-ECG could not detect AF recurrence at 6M-PVI in 19% of PAF patients. (2) Around half patients with early-AF recurrence (1M-PVI) had no AF at 6M-PVI. (3) Among no-AF patients at 6M-PVI, AF was detected in only 2% at 12M-PVI. Whereas, among patients with AF recurrence at 6M-PVI, AF disappeared in 11% at 12M-PVI. (4) Their symptoms have little relevance to AF recurrences. (5) For PAF patients, mean heart rate during sinus rhythm was significantly increased after PVI. The tendency continued for 12M-PVI in non-recurrence group but did not in recurrence group. <B>Conclusions:</B> Long-term follow-up after PVI might be required since the AF recurrence rate gradually decreases with time. Mean heart rate during sinus rhythm might be a good predictor for late-AF recurrence.
