Fourteen symptomatic arteriovenous malformations of various sizes were surgically removed without any operative death from the vital cortical areas of cerebral hemispheres which were localized by the Ring-Waddington drawing system in the carotid angiograms. Out of 14 arteriovenous malformations, 8 were located in the motor-sensory region, 2 were in the speech region, and the remaining 4 were in the dominant parietotemporal region. Radical extirpation caused temporary disabilities in every patients just after the operation. Those postoperative neurological deficits, however, improved in all but a patient and full recovery to total working capacity and recovery to modified working capacity took place in 93% of the patients.<BR>The meticulous radical extirpation is thought to be the method of choice even in the treatment of arteriovenous malformations in the vital cortical areas when they bled, especially when they repeatedly bled.
