Tympanometry is useful in the evaluation of the middle ear status in various ear diseases. It has been believed that if the drum is perforated, the tympanogram (TG) becomes a straight line due to the lack of compliance change in response to the pressure change in the external auditory meatus (EAM) provided that the applied pressure does not leak through the eustachian tube.Recently the authors found two types of TGs with distinctive characteristics in ears with small perforations. In the first TG type, when the EAM pressure was decreased, a compliance peak appeared in the positive pressure area, whereas when the EAM pressure was increased it appeared in the negative pressure area. The difference in the pressures at which the compliance peak was obtained was great.The second TG type was unique with multiple notches similar to those of ENG recordings in jerky nystagmus. Both TG types were observed in ears with very small perforations, having granulations or small amounts of fluid covering it, serving as valvelike structures.Knowledge of these TGs is helpful in recognizing small tympanic membrane perforations. When TGs with positive middle ear pressure are obtained, small perforations should also be suspected as well as the initial stage of acute otitis media.
