Chronic Tophaceous Gout in Woman A Case Report

Gout is commonly seen in men and recently chronic tophaceous gout of severe degree is rare. We report our experience of managing a 42-year-old woman with chronic tophaceous gout and aseptic necrosis of the left femoral head. Laboratory data showed hyperuricemia (serum uric level 10.7mg/dl) and renal insufficiency (BUN 45mg/dl, creatinine 4.6mg/dl). While being treated with allopurinol (300mg/day), the patient developed high fever, skin rush of the trunk and extremities, and agranulocytosis. Allopurinol was discontinued with apparent resolution of them. She is now treated with benzbromarone (100mg/day), but hyperuricemia is still observed. We consider that allopurinol desensitization may be necessary for this patient.
