A case of multidirectional instability of the shoulder joint after the first traumatic dislocation in childhood.

It is well known that the younger the age of the patient at the initial dislocation of the shoulder joint, the more frequently the dislocation recurs. Rowe reported that all cases sustaining the initial dislocation below the age of 10 have frequent recurrence of the dislocation later. We report a 12-year-old boy who had recurrent dislocation of the shoulder joint due to multidirectional instability following the initial traumatic dislocation of the shoulder joint at the age of 3. On admission, his right shoulder joint showed multidirectional instability and muscle atrophy around the shoulder. No neurological abnormalities were seen. Radiologic examinations revealed inferior instability and slipping of the humeral head. Double contrast arthrograms demonstrated enlargement of the capsule and Bankart lesion. The glenoid osteotomy and capsular shift resulted in a satisfactory outcome.
