A rare case of paralysis of the deep branch of the radial nerve caused by extostosis of the radial head is reported. The patient was a girl aged twelve, who noticed disability in extention of the right thumb and fingers without sensory disturbance. A palpable lump at the radius head was present and was thought to be a cause of symptoms. Examination revealed no additional abnormality in other bones, but electromyographical changes. Ulnar part of the tumor of the radial head restricted supination of the forearm, and radio-dorsal part was thought to have compressed the deep branch of the radial nerve. The tumor was resected from the radial head. The tumor was histologically diagnosed as osteochondoroma. Function was completely restored clinically 8 weeks, and electromyographically 10 weeks after operation.
