A case report of pseudomalignant osseous tumor of soft tissue at right transverse process of the third lumbar vertebra.

A school-girl aged nineteen complained of low back pain and stiffness without a history of trauma. Three weeks later severe pain and swelling were found on the right paralumbar spine area. X-ray showed no pathological findings, but CT scans demonstrated a lesion like periosteal reaction at the level of the right transverse process of the third lumbar vertebra. We suspected of the findings of malignant tumor from 99mTc bone scans, angiography and MRI. But after 7 weeks the low back pain diminished, and CT scans showed the characteristic bone shell. At operation the lump was found approximately five cm in diameter on the right transverse process of the third lumbar vertebra. Histologically, the feature of this lesion showed no malignancy but the characteristic zone phenomenon which consisted of a central undifferetiated zone, an intermediate zone and an outer zone of maturing bone. The lesion was diagnosed as pseudomalignant osseous tumor of soft tissue, described by Fine and Stout (1956).
