Biweekly therapy for non-small cell lung cancer using cisplatin, paclitaxel and gemcitabine

Purpose : Combination chemotherapy using a platinum and one new drug has become the mainstay for the treatment of non-small cell lung cancer (NSCLC), however, the evaluation of triplet combination therapy has not yet been established. The purpose of this study was to evaluate the response rate and toxicities of triplet combination therapy by administering cisplatin (CDDP), paclitaxel (PTX), and gemcitabine (Gem) every other week. Patients and Methods : Twenty NSCLC patients, 18 men and 2 women (mean age : 66.2), were enrolled. The histological diagnosis was squamous cell carcinoma in 10 patients, adenocarcinoma in 8, adenosquamous carcinoma in 1 and alveolar cell carcinoma in 1. The stage was IIIb or IV in 14 patients. Six patients had previous treatment. CDDP, PTX and Gem were administered every other week at 30, 120 and 1, 000 mg/m^2, respectively, however these doses were adjusted taking various factors into account such as the patient's age performance status and toxicities of grades 3 and/or 4. In principle, 4 cycles of treatment were conducted as a rule. Results : Hematologic toxicity was seen most commonly. Severe anemia in 3 patients and neutropenia in 1 patient were also observed. Other grade 3 or more severe toxicities included weight loss in 1 patient, nausea and vomiting in 2, hyperkalemia in 1 and hyperuricemia in 1. The therapeutic response was complete response (CR) in 2 patients and partial response (PR) in 7. Surgery was conducted after chemotherapy in 5 patients (curative resection in 4). Conclusions : The therapeutic efficacy resulted in a response rate of 45%. Treatment was not discontinued in any patient because of serious toxicity. Further studies are warranted on other aspects of this therapy such as the resulting prognosis.
