Depressed Oxidative Function in Gingival Crevicular Polymorphonuclear Leukocytes from Porphyromonas gingivalis-detectable Periodontal Pockets

本研究ではPorphyromonas gingivalis可溶性成分が末梢血(PB)及び歯肉溝滲出液(GCF)中の多形核白血球(PMNLs)のoxidative burstに及ぼす影響を調べた。最初にIn vitro実験として健常者のPB PMNLsを,歯周病関連細菌として選んだP. gingivalis, Prevotella intermedia, Fusobacterium nucleatum, Campylobacter rectusの4種の培養上清で培養を行った。その後,Phorbol myristate acetate (PMA)あるいはformyl-methyonyl-phenylalanine (FMLP)で刺激を行った場合の,それぞれのoxidative product (2', 7'-dichlorofluorescein; DCF)産生量をFlow cytometryにて測定した。その結果,P. gingivalis及びP. intermedia培養上清で培養した場合は,PBS培養に比べて,いずれも有意にDCF産生量が増加した(p<0.001)。次いで,上記の4種の細菌培養上清で培養した後に,PMAあるいはFMLPで刺激した場合,P. gingivalis培養上清の場合にのみ,DCF産生量に有意な減少が認められた(p<0.01)。また,In vivo実験としてP. gingivalisが検出された歯周ポケット(P. g.^+ sites)と検出されなかった歯周ポケット(P. g.^- sites)からcrevicular washingにて採取したGCF PMNLsのDCF産生量については,P. g.^+ sitesとP. g.^- sitesの間で,培養可能な嫌気性菌数,GCF PMNLsの回収率(recovery),純度(purity),生存度(viability)などほぼ同一な条件の中で,PMAまたはFMLP刺激した場合のDCF産生量についてはP. g.^+ sitesのGCF PMNLsの方がP. g.^- sitesの場合と比べて有意に減少した(p<0.01 to p<0.05)。以上の結果より,P. g.^+ sitesでのGCF PMNLsのoxidative functionが抑制したのはP. gingivalis可溶性成分による過大なoxidative burstに起因していることが示唆された。
