A case of spontaneous urinary extravasation from the renal pelvis following loop diversion is reported here. Initially, it was believed that there was leakage from the site of uretero-ileal anastomosis, unilateal or bilateral, but IVP disclosed a massive extravasation of dye from the bilateral renal pelvis. The patient was diabetic and had a small urinary stone in the right ureter. Right ureterolithotomy followed by control of diabetes cured this unusual status.
