Clinical Application of Diffusion-weighted Magnetic Resonance Imaging to Intracranial Disorders

各種脳疾患における拡散強調画橡(DWI)の臨床応用について検討した。脳梗塞19例、脳出血5例、神経膠腫4例、髄膜腫4例、水頭症4例、慢性硬膜下血腫5例を対象とし、患部の拡散の程度及び、脳梁・内包後脚・視放線での水分子の異方向性の検出度を検討した。脳梗塞急性期では拡散遅延、慢性期では亢進した。脳出血では血腫は拡散遅延、周辺浮腫は拡散が亢進した。水頭症では傍脳室部で拡散が亢進した。脳腫瘍では髄膜腫、神経膠腫の周辺増強域で拡散遅延、周辺浮腫では亢進した。また拡散の異方向性は、内包後脚での検出率が最も悪く、患側で著明であった。拡散強調画橡の臨床応用により、病態の形態学的把握のみならず、動的・機能的把握が可能と思われた。
