Giant Cell Granulomatous Hypophysitis Manifesting as an Intrasellar Mass with Unilateral Ophthalmoplegia : Case Report

左動眼神経麻痺で発症した下垂体巨細胞肉芽腫の一例について報告する。症例は62歳女性。頭蓋単純撮影では、軽度のトルコ鞍の拡大と鞍背のerosionを認めた。MRIにてトルコ鞍内に限局した脳と等信号の腫瘤を認め、Gd-DTPAで均一に増強された．腫瘤は経蝶骨洞的に全摘され、組織学的に巨細胞肉芽腫と診断された。術後10日間の経過で動眼神経麻痺は消失した。下垂体巨細胞肉芽腫は症候学的および放射線学的に下垂体腺腫と鑑別困難だが、腺腫に比べ視力視野障害を生じにくい傾向にある。トルコ鞍およびトルコ鞍上部の腫瘤で、視力視野障害のない動眼神経麻痺を認める場合、巨細胞肉芽腫を鑑別診断に加えるべきであると思われる。
