EFFECTS OF LOW-CALORIC DIET IN PUERPERlUM ON PROLACTIN, TSH, ESTRADIOL AND MILK SECRETION

概要産褥婦人33名中,10名に低カロリー摂取群(LC群)として分娩当日(0日)より3日間低カロリー食(1,200 kcal/日)を与え,続いて3日間常食(1,800 kcal/日)を与えた.15名は常食摂取群(OC群)として0日より6日間常食を与えた.これら2群のプロラクチン(PRL)値,TSH値および母乳量の経日的変化を比較検討した.更に両群各々4名に1日と3日にThyrotropin-releasing hormone(TRH)を投与し,PRLとTSHの分泌反応を比較した.その結果,OC群のPRL値はLC群のPRL値に比し高値を示し,特に2日には有意差(p<0.05)を認めた.このPRL値に反映する如く母乳量もLC群に比しOC群は常に高値を示し,同様に2日に有意差(p<0.01)を認めた.TRHによるPRLの分泌反応はLC群に比しOC群は高値を示したが両群間には有意差は認められなかった.両群におけるTS耳の経口的変化は認められなかった.しかし,TRHによるTSH分泌反応はLC群においてより抑制傾向が認められ,特に3日のTSHピーク値は有意(p<0.05)に抑制された.以上の結果,低カロリー食はPRL,TSHおよび母乳の分泌を抑制するものと思われた.
