EVALUATION OF DNA PROBE TEST FOR CLINICAL DIAGNOSIS OF CHLAMYDIAL AND GONOCOCCAL INFECTIONS OF THE UTERINE CERVIX

クラミジア・トラコマティスによる子宮頚管炎の診断においては, 培養に代りうる簡易でしかも感度, 特異性の優れた抗原検出法が必要である. 従来広く用いられてきたクラミジアザイムは, ある種の細菌との抗原交叉性により偽陽性がみられるという問題点が指摘されている. そこでわれわれは近年開発されたDNAプローブ法(Gen-Probe)の臨床診断における有用性を培養法に基づいてクラミジアザイム法と比較検討した. 当科外来において95名のハイリスク患者より127検体を採取し, それぞれに培養, クラミジアザイム, DNAプローブの3法を施行した. 培養法に対しDNAプローブ法の感度は78.4%, 特異性は93.4%であり, クラミジアザイム法ではそれぞれ90.2%, 86.8%で, DNAプローブ法はクラミジアザイム法と比較し感度は劣るが, 特異性が高いという結果であった. さらに本法には同一の検体を用いて淋菌による感染も検出できるという利点があり, 75検体について調べその結果を淋菌培養と比較したところ, 3検体がともに陽性, ほかはともに陰性で不一致例は存在しなかった. 以上より今回検討したDNAプローブ法はクラミジアおよび淋菌による子宮頚管炎の臨床診断において有用な手段になるものと期待されるが, クラミジア検出感度における改良が望まれる.
